Hello sir,
I am Sweety Parakh Shivam Parakhs sister.
I am not able to receive my claim form which has been told that the insurance department has already made the courier but I have not received and i am following up from the past 8 days and the matter has been escalated to the doctor's too .
I am staying at Vapi Gujarat and was told that I will be made the courier of the claim form.
Hope I get a solution.
