Complaint Against Reliance Communications Netconnect 2G CDMA datacard

I had been using a Reliance Netconnect 2G CDMA datacard with number 7483036907 for several years. However, since the second half of June 2016, the datacard stopped working (I came to know later that this occurred because Reliance stopped offering CDMA services). I made several phone calls to Reliance customer care to fix the issue, but could not find a rational answer.

As such I switched over to a new service provider. Because I did not require the Reliance datacard anymore, I tried to request a service disconnection. I made several calls to Reliance customer care, but the agent at the end of the IVR never transferred my call to the right team.

One of the agents asked me to visit the nearest Reliance Store to request a disconnection. Accordingly, I visited the nearest store in BTM Layout, Bangalore, but the staff there advised me to call up customer care instead. Back to square one. Even after repeated attempts, I was unable to get the service disconnected. They never issued a complaint number either. I called up customer care again, but again my call was not transferred to the service disconnection team.

As a last resort, I sent an email to the Reliance Netconnect customer support asking for a disconnection on 1 July 2016 (email attached). To my knowledge, I had paid all my dues until the point when service issues occurred. However, my service was not disconnected and Reliance kept sending me bills and has been harassing me with phone calls. I sent a second email to the Reliance Karnataka Appellate Authority on 29 August 2016, asking for a disconnection (email attached). But I did not receive any acknowledgement or complaint number. They kept harassing me with phone calls and emails.
On 24 September 2016, I received a demand notice from Reliance stating that an amount of Rs.1484 is overdue on my account and that legal actions may be initiated if I fail to pay. To avoid issues, I made the payment on Mon 26 September 2016 (payment acknowledgement attached). At the same time, I also sent a 3rd email to Reliance customer support and Karnataka Appellate Authority to disconnect my service and stop sending me bills (email attached).
However, on 9 October 2016, Reliance sent me another bill stating that amount of Rs. 254 is overdue on my account.
So far, I have not received any acknowledgement from Reliance regarding my requests for service disconnection. There is no explanation for the charges levied. I have been harassed repeatedly with bills and phone calls since July 2016. I request your kind intervention to help sort this issue and make Reliance pay adequate compensation to me for the mental agonies I had to go through all these months.
