Hello sir,

I am Sweety Parakh Shivam Parakhs sister.
I am following up with the insurance desk department from past 8 days regarding the insurance claim form . But no one is attending my query.

I have to submit the document to the insurance department and due to the non availability of the form I am not able to submit the document.

I am staying at Vapi Gujarat and it won't be possible for me to go to ahemdabad.
I was told that the claim form has been couriered but still no answer has been received.

Requesting your intervene in the matter.

    Apollo hospital has written some of the places that no mask no entry ...now visitors has no idea about this ...and there they are selling disposable mask for rs.10 right next to dr.s cabin ...it seems like they and doing mask business by writing this ...if you dnt allow patient without mask then this is basic needs to provide mask to them free ...but instead they charged rs.10 as a income source ....when. I said the assistant there for free then the reply was lena hai toh lo nahi toh jao
