refund of mobile hand set due to unsetifactory service

I have submitted my sony M4 mobile hand set to your service station on 14.04.2016 wide ticket no-16041402834.
I have purchase this mobile on dated 20.08.2015.
first time repaired by your service station on 21.09.2015 with 8 days repining time.
second time repaired by your service station on 09.02.2016 on same day.
this is the third time my mobile goes faulty and it was submitted on 14.04.2016 still not repaired.
it means within 8 month the mobile is repaired three times.

To day I have visited sony service centre RAJKOT, I and the service centre in-charge has checked the mobile handset and found that is is not repaired. So, the in-charge has advise that it will be sent again to HVC Ahmedabad for repairing.

Now it is too late for repairing service. I am totally dissatisfy with your mobile hand set and its repairing service.
So, I want my money back i.e. REFUND of paid bill amount otherwise I will go to consumer forum.
