I was under treatment for CHRONIC LIVER DISEASE-CRYPTOGENIC, NONCIRHOTIC POTAL HYPERTENDION at CHRISTIAN MEDICAL COLLEGE VELLORE.All treatment incluing pathological and radi0logical
refered by Dr online.No prescription is given at CMC,VELLORE.Only online lab order prescription and OP referal.Lastly CMC,Vellore provided me MEDICAL REPORT giving all details of pathological/radiological and advice bearing Hospital No 158337H.
United India Ins. Co has wrongfuly paid me Rs4353/ with objection(a)DR ADVICE FOR INVESTIGATION DTD 05.04.18(b)DR ADVICE WITH REPORT AGAINST BILL DTD 23.04.18(DR PRESCRIPTION DTD 23.04.18
Further all online lab order,prescription and referal with money receipt were enclosed with claim form.
May I request you to take my complain with UNITED INDIA INS CO LTD and do justice to me
