Dear sir/madam,
My self MR. Rajaram pd. gupta.i want to say that my motorcycle (i.e BR32C 9854) XCD 125 who is accident on the dated of 31.12.2013 and in this accident i was also very injured and i was gone DMCH Hospital to the help of another person and DMCH's doctor suggest me take the bed rest 1.5 Month. After than i submitted the application in the Branch office National Insurance Company, Madhubani Dated 22.01.2014 thure my younger son (Sateesh Kumar)every proof attached with given below.
