Complain against YOU Broadband India Not installed phone connection and delay in Internet connection

I applied for internet connection & a phone connection (which they were providing in tie up with Vodafone as it was or commercial purpose) to YOU Broadband India Limited on 05/August/2016. there was no response from the team for a very long time about the connection. I was calling them again & again during all this period but thee was no response. I had already given the cheque to them so i didnt understand reason for delay. All this while i was talking to Mr. Amit (guy who took the cheque) & Mr. Naveen (whos no Mr Amit gave saying he looks after installation). Both of them never used to pick my call But they would pick when called from unknown number & used to say its in process we will install in few days. I even called customer care twice, in which we told the situation & that we are not getting the service. They said they will look into it & also gave reference no. which was never resolved. The reference no. are 2016092628570 & 2016093015039. After all this later in September they said there was some problem and we will install it in few days & the cheque got cleared on 28/Sept/2016.(which you can verify from the bank statement in attached document)
Even after taking the money same behavior was seen in them, not picking calls & not providing service. Then my friend went to their office at Shivajinagar Pune on 1/october/2016 & met Mr. Narender Patil, who was senior on Mr. Naveen & he assured that installation will be done within few days but even after that it was not done after constant calls & follow up to all 3 guys whos name i have mentioned above.
Finally after many calls and follow up they installed internet connection on 15/October/2016, But they did not install Phone connection which was earlier agreed upon.

And till date they have not installed phone connection & they do not pick up our calls or revert to calls.

And all this while they never called or came to provide service from their own. it was always after our constant followup.

I am tired of following up them for over 2 months now. It's been a traumatic experience.
I need full payback which i have paid i.e. amount of Rs. 5946/- also compensation for all the mental harassment i went through all this while to get a simple service for which i have already paid.

Please help me with a solution.
