amount debited from my saving account but cash not received at ATM

On dated 13.03.2016 I had visited an Axis Bank ATM located at Ashoka Enclave, Sector-35, Faridabad for cash withdrawal. I had withdrawal Rs. 5000 but in saving account (No. 10724955320) amount debited two times for Rs. 5000 and Rs. 10000 at the time of 7:42 PM and 7:49 respectively.
