Vodafone excess billing & service request not processed

Hi,
I’m Dr. Chethan J.D. as cutomer of Vodafone/Hutch since ~9 yrs.
1. I’ve moved to my new place of work Dharwad-Karnataka from Tumkur. The problem of network cross-over have not been resolved. Also, the network quality is very, very poor.
2. October, 2015 that suddenly my plan changed and was asked to pay a different slab as the previous plan was no more valid with the same offer I till that date used to have. without any communication.
3. June,2015 I suddenly I got a bill in excess of Rs. 1500 without using anything much of that kind.the detail e-bills were never communicated to me. But the plan was once again changed and this time without any intimation or message or a communication of any kind got frustrated for paying repeatedly Rs. 1100+, Rs. 1500+ and Rs. 1700+ & Rs. 2400+ for August,2015 still continued.
4. In the mean time, requested for plan change on 9886489565 & 9886427923 (Vodafone red family plan of 899+taxes). I also wanted to add my aunty Ms. Gayatri H. (Mob. No. 9886677465, at Chitradurga). One more is she got post paid conversion on a different number (9886677503).
5. At Dharwad, I requested activation of Vodafone red plan, at Vodafone store. I could not get a clear explanation to why should I pay till the subsequent bill cycle for a rental plan in excess of the usage (about the other two numbers 9886427923 & 9886677465, as they were in minimal). But on bill I find that they both have been activated into Vodafone red individual 899 & 499 plan, I got the charges billed as per Vodafone red individual plan rentals. As till date I have been calling them, (innumerable having called the customer care and higher people, every time-no solution) at Vodafone store I’m facing the same problem.
6. Later, the bill was Rs. 2845.91+Rs. 1062.96 and Rs. 470 odd on Ms. Gayatri’s 9886677465 number. I would like the concerned authorities take this seriously and solve the long standing issues and payments done in excess on 9886489565, 9886677465 and 9886427923.

In continuation the company has apologized for the mistake but has not accepted my claim for the amount I've paid in excess and trouble caused in the sue course. I was not assured of any refund of any excess amount paid but was told about not repeating the mistake again, which was repeated again recently and also this time was apologized and refunded-mail reply as below;
