POLICIES SOLD FRAUDLY IN THE NAME OF INSTALLATION OF DDTM MACHINE OF AIRTEL

This mail in reference to policy issuance of Hdfc life insurance policy No—



1. 2468847-- Survival Benefit Payout/ HDFC Life Sanchay Par Advantage ------issued on 1st March for Rs. 106000/-

2. 24608512- HDFC Life Super Income Plan --- issued on 2nd feb for Rs. 29000/-



The above policy has been issued by the Airtel against the installation of DDTM machine.



I received a call from Mr. L N Swami from airtel (Survey Team) on 2nd jan 2022 & said that we have identified your location for the installation of D.D.T.M machine from bharti aitel at your premises 70- ishwarpuri sector 12 Indira Nagar lucknow.After that I received a call on 11th jan & he whatsapp me to send the following documents at the below mention location for the registration of site:

1. Photos( 2 photos)

2. Pan card

3. Adhar card(1 copy)

4. Cancel cheque

5. Amt chq in favour of hdfc life insurance co ltd for rs.29000/-

After that they have issue a policy in my name App no-1470071329069 of hdfc policy( All screen policy is attached for your ref)

He informed to me that you will receive a rent of Rs. 30000/- every month from airtel ( first six month rent in adv. ) & after six omth monthly rent will be credited to your account with addiotnal incentive of Rs. 2000/- per machine over an above 15 machine frequency level for a tenure of 25 years



On 1s t feb Mr. L.N Swami has informed to me that you will I receive a call from HDFC life by vivek sharma for confirmation of policy at the time of confirmation Mr swami(Airtel Survey team) has informed me that insurance co will ask that is this policy is issued in regard to any tower installation then you will say NO . After that I am receiving a fake call from the brokers then Mr. Achal Awathi (Airtel Relationship Manager Assistant) has take up the call & informed to them that this policy has been issued by Airtel .



On 14th feb they have whatsapp me that your file is updated on server Mr. Abhishek chauhan(Relationship Manager of bharti Airtel) will call you shortly he has all the information of your process.



Mr. Abhishek Chauchan( Bharti Airtel –Relation Ship Manager) has called me that we have selected your site for installation of D.D.T.M machine against which your will receive a monthly rent of Rs. 30000/- per month ( including six month rent in Adv )after six month monthly rent plus additional incentive of Rs. 2000/- per month above 15 machine frequency level. Every year 6% annual increment will be in your rent amt. the Rent agreement will be made in civil court by our lawyer & duly signed by you & bahrti airtel which is registered also (original copy of rent Agreement will be retained by Airtel). One contract Agreement will also be made for 25 years between us whose original will be kept by you & xerox will be hold by Airtel. As per Mr. Abhishek Chauhan the above process will be completed within 90 days from the issue of UIN NO & selection letter. He said this polcy will taken by you boz the first year the co is not earning any thing ( for life insurance cover of 30 lac) from next year our co will start earning from these machine & will pay the premium. The above policy will be under mortgage to Airtel (docs attached). At the time of agreement signing our lawyers will give you the Xerox copy of the policy



The Airtel will not charge any amount. He siad that you can call to me at any time & has given me his assistant t no ---Mr. Achal Awathi-mob no 7398496724. As per him all our call are on server & rest other thing will be corordinated by my Asist MR Achal Awasthi. Mr. Abhishek Chauhan has given my Reg No- TNIAOOOPZ5666/2022 LANGUAGE CODE- CHLP6762 which is not to be shared with any one as some fake call will be received at your end & they will ask for the same.



After that Mr Achal Awathi & Mr. L N swami started communicating with me. On 15th Feb Mr. Achal Awasti has called me that we are searching for contractor ship location in lucknow, then I suggested him sir my house is also vacant & is in prime location if it suits you then plz go for it, thereafter they have asked me to share your house video, I have shared him my house videro & he confirmaed me that it suits to them . Mr. Aachal Awathi siad that for contractorship you have to take the polciy of 135000/- of which your amt of RS. 29000/- is already deposited with us & bal amt of Rs. 106000/- policy of HDFC is issued.(policy copy is attached)



For contractorship the following terms & condtions are brefied by Mr. Achal Awasti (Asst of Mr. Abhishek Chauhan---Airtel relationship manager) Mr. Achal Awasthi Emp code—CS0000055999.



1. Rent amt will be Rs. 58930/- per month

2. Rent increment after 5 years.

3. Upto 3% for 10 years

4. After 10 years 4.5% for 17 years

5. Upto 5.1% for 26 years

6. Afeter 26 years 5.8% upto 33 years



Initally 25 mahine will be installed under your contarctorship



We will provide one person job for Rs. 15000/- per month after 15 months there will be incremant of 5% in salary.



Secutiy deposit of Rs. 15 lac will laso be credited into your account.



On 23rd feb they have informed that your location is now finalized for contractor of DDTM of lucknow for contractors ship

On 24th feb they have share me the Application for registration of contractors of bahrti Airtel which is attached for your ref . the contractor annexure i have filled it properly & send to Mr. LN swami which is attached .



On 1st march the policy is issued Survival Benefit Payout for Rs. 106000/-. This policy confirmation call has been received from the HDFC call centre & I have merged the call to them & they have informed to them that this policy is issued by Airtel.



From ist march I am in regular touch with airtel team for the installation of DDTM machine , but every time they give some excuses. But till date no machine is installed & after 10th June there Nos are switch off & after 1st July they have blocked my Nos.



You are requested to plz cancel these policies on priority basis. All supporting docs in which policy is mortgaged are attached .

I have also lodged a complaint to the Airtel Ombudsman.
