Lab Test Package charged but incomplete tests

Hospital Complaint Complaint Date: March 13, 2016
The attached Lab test package was chosen by me in which AMH(Anti Mullerian Hormone) test is included but there is no test report included in the attached report. I have chased many times to SRL neither they are refunding the money for this.Also this test can be conducted on a specific day for a woman and now it can be only effective a month after.
