Dear Sir,

My father(SADU NAGESWARA RAO) Was a lorry driver when he was in duty from vijayawada to jaleswar during the jouney his health was not good,so he was admitted by Balasore DHH with the support of cleaner. on the same day( 07/04/2018) he was died in hospital(Reg NO.654/2018) but at the time of registration there clerk done a small mistake actual my father name is "Nageswara Rao" instead of that he entered as "Najeswararao" but still today i didn't received the death certificate from your state due to the clerical mistake happened at hospital. So if possible means kindly do rectification as per My father's AAdhar card and provide me death certificate as iam non resident of Odisha state..Iam from Vijayawada,AP.
