Poor Service visit to the branch to get ATM card

Service of this branch is really very poor.They don't respond to the problems we have nor they provide any reciept for any form submitted there. If we ask for any form/service they jusy say staff is not available come next week.
From last 6 months,i am trying to get my ATM card reissued but I am disappointed every time I go to the bank.Some times they say, we do not have staff, some times they say, we don't have forms fro ATM card. I am a working women, so every even Saturday, I visit to the branch to get ATM card, as I am unable to use the amount stuck in my account.
Finally I decided to change my branch, I filled and submitted the form. They have not provided me any receipt, so I have no proof of that, neither I can go and say that plz see that request again.Its been more than month, they hav not changed my branch nor provided me my atm card.I really dont understand what should i do now .
