Generated prepaid Bill after requesting disconnection

I had requested for disconnection on 1st or 2nd week of 10/2015 ,But they had generated Bill on 26/2015 for 11/2015 and requesting me to pay amount 497/-
Moreover I did so many follow ups on disconnection request but no-response and email them for updates still didn't responded me, But they torturing me with calls and emails to pay 497/-
