TATA SKY not refunding balance amount

Sir,
I am a subscriber of TATA SKY vide id no 1145275580,& recently I have shifted from Nashik to Bhubaneswar where I have lost my setup box. I have requested a new setup box which they are not supporting me, also I have asked them to refund my balance amount that also they are not refunding to me.
Kindly look into the matter.
