my form 4 for joining pahal yojna is been miss palced by agency iam not been able to recieve subsidy

I am an indian gas holder my consumer no is 2844 .I have a indian gas connection by mamta indane gas agency baniyapur .i had full fill the form no 4 for the subsidy but iam not been able to recieve my subsidy after getting two indiane cylinder .when I search online on my lpg it say iam not been join in the pahale yojna i will not been able to recieve subsidy. So i want to know how i will recieve subsidy
