Gas Agencies in Machilipatnam, cylinder delivered had a problem with washer

I am D.Chandra sekhar sastry with consumer no: 734008 of HP Gas machilipatnam at 23-123,shaikh email road, machilipatnam.The cylinder delivered had a problem with washer as confirmed with the technician and is been leaked since started using and now the cylinder became empty in 2 days.Do you really check the quality of the cylinder before delivering?who is responsible if any threat happens because of gas leakage?Also I lost 1 cylinder amount without using.
