bill receipt not given by cable operator

in my house we have DEN network cable connection bearing no 000090251869 .i asked cable operator bill for set top box and monthly charges which is 200/month and 1250 for set top box but he Deny then I complaints at den cable customer care no 18004192020 with complaint reference no 3668072 but they also deny for bill and send warning massage for cable disconnections if bill not paid with out bill receipt.
