Issued Termination Voucher instead of Transfer Voucher

My name is Manoj Kumar and I had a gas connection in Srajan HP Gas Distributors, Udaipur. Since my job was recently transferred to Ahmedabad, I had to relocate my family there and hence I went to Srajan HP Gas for transferring my gas connection to Ahmedabad. I am into the private job for last 14 years and have worked in various cities of Rajasthan, Haryana, Punjab and now in Gujarat. Whenever my job was transferred to another city, I used to go to the concerned Gas agency and got my gas connection transferred to next destination city.
On 13th April 2019, I went to Srajan Gas for the same purpose. A boy was sitting in their office who was not aware of the process of transferring gas connection so he said that let his madam come, she will do the needful; after around two hours wait a lady came and I explained to her that my job is shifted to Ahmedabad & I need to transfer my gas connection to Ahmedabad. She said that I am their first applicant who has come for such a transfer case. It seemed that she also was not aware of the process of transferring a gas connection, therefore she called someone over the phone and explained my case to him/her, she was asking the other person about my case that what to do. After disconnecting the phone call, she asked me to surrender the gas cylinder and regulator. I agreed to the same and requested her to search HP gas agency in Bopal (Ahmedabad) area for me where I can take connection based on my existing HP connection. Then she called a couple of numbers to check the same and finally informed me that there is no HP gas in Bopal area. Then I requested her to keep the option open, so I can submit my gas voucher in any agency and take connection. Finally, they issued me a voucher and returned me some money after taking my two gas cylinders and regulator. She asked me that wherever I will go, I should submit this voucher to the concerned gas agency and they will give you two cylinders and regulator.
On 16th April I went to Amardeep Bharat Gas Agency in Bopal (Ahmedabad) and submit my all papers along with the vouchers and security deposits which were issued by Srajan HP Gas. The person sitting at Amardeep gas informed me that tomorrow cylinders and regulator will be delivered to your home in Ahmedabad along with the Gas book. Next day we received the cylinders and regulator but did not receive any Gas book. After few days I called Amardeep Gas agency person and informed that I haven’t received my connection papers yet, he informed me that my TV is showing error on their system so they are not able to generate my papers from their end. I requested him to speak to Srajan HP Gas and have the needful done; he said ok he will speak and get the needful done. Thereafter I had no communication from both the agency. After a long wait, on 14th June 2019, I went to Amardeep Bharat Gas agency and asked the status. Lady sitting there informed me that my Adhar number has to be deleted from the previous voucher and then it will be displayed on Bharat Gas's system. Then that lady called Srajan Gas agency over the phone and explained the lady (in Srajan) about deleting the adhar number. Still, that lady was not aware how to delete the adhar number and she said that she is getting the same done from their head office jodhpur, she said it will take 15 minutes. After a few minutes, Srajan Gas agency lady called and informed me that my adhar number is deleted from their system.
Then I requested Amardeep Gas agency lady to recheck in their system, but still, she was not able to see it. Amardeep Gas agency lady called her senior and explained about my case and she came to know that I was issued the wrong voucher. In place of Transfer Voucher, Srajan Gas Agency issued me a Termination Voucher and this was the reason that Amardeep Gas Agency people were not able to process my connection.
Thereafter, I spoke to Srajan Gas agency’s owner and asked him to resolve the issue. Since they issued me a wrong voucher because of their inefficiency and lack of working knowledge, Mr. R K Jain (Srajan Gas agency’s owner) tried to hide their mistake. He said I only asked them that I want to surrender my connection and that is why they issued Termination Voucher. I tried to explain to him that I have been working in different cities of the country for last 14 years and everywhere I have got my gas connection transferred; I never asked them to terminate my connection. But he was not ready to accept this and kept disconnecting my calls in between. Mr. Jain also mentioned that transfers can only be done between HP to HP Gas and not to other Gas agency, this was quite surprising for me since I have been getting connections in different-different agencies everywhere.
Here my earnest requests to the Consumer Court to–
1. Please check that how many Transfer/Termination Vouchers have been issued in Srajan Gas agency before my voucher. I trust mine was the first one and lady was not aware of the process, hence she issued me the wrong voucher.
2. Ask Srajan Gas Agency to immediately issue me a Transfer Voucher so I can take connection in Ahmedabad.
3. Ask Srajan Gas Agency to pay me Rs. 50,000 as a settlement amount against the mental and physical trouble I had to go through because of inefficiency and lack of working knowledge in Srajan Gas Agency Staff.
