Insurance Policy Number- 321602/31/16/6100000534
Policy issuing office: Station Road, Balaghat, Madhya Pradesh, Pin 481001

I met a minor car accident on 29.10.2016 and the accident damage report of the same is intimated to Balaghat office of National Insurance Company. Whereas vehicle's survey and damage repair and maintenance is carried out in Raipur (C.G.) during period from 08-Nov-2016 to 13-Nov-2016 and relevant documents were submitted to the surveyor Mr. Gagan Chopra (Mo-9425581553). But till date no claim payment is settled and on verbal discussions with my Insurance agent Mr. Bairagi (Mo- 9425581553), he replies Balaghat office has not received the claim documents where as on telephonic discussion with Raipur office personals of National Insurance Company limited they reply they have sent the documents.
I even tried to put online grievance complain in national Insurance company’s web portal, but it made claim number a mandatory field without which it do not process further, due to non-availability of claim number I cannot register a complain in the same.
Residing in a remote location like Malanjkhand, it is not possible for me to visit Balaghat or Raipur for search of documents again and again.

Please do the needful.
