Hospital is not providing the test report for the diagnosis they have mentioned

My mother was admitted in Balaji hospital Byculla, Mumbai with the complain of severe cough.
They conducted certain test and diagnosed COPD.
My insurance company (HDFC ERGO) they need the valid documents for the claim process.
They had already troubles at the time of discharge as they did not provide the valid documents and reports to the insurance company.
My parents were in the hospital for 3 days after the discharge as there was no one to help with the process and the bill amount was 1,47,000.
After loaning the money from some one we paid the bill.
Now to claim that money from the insurance company they are asking for the diagnoses reports and the hospital is unable to give the same.
We are calling them and also visited the hospital but there is nobody to help and nobody has any answer or solution.
My mother is retired already on bed due to surgery on her leg and she is a heart patient recently been through engioplasty.
Please we need some help.
