New gas leakage

Dear Sir
I would like to inform you that my new gas cylinder found leakage when I change the empty cylinder. This is the repeated issue happened with me. Two times I informed to gas delivery concern person, now today is third time. Why the leakage issues is reported? Please take necessary action to avoid leakage of cylinder & accident. This is very serious issue.
