With the reference we state that one of our employee Mrs. Adhuri Paramanik bearing PF A/c No 1103 & UAN No- 100073903023 want to withdrawal her PF in advance, however, at the time of vetting her claim for withdrawal following inconsistencies are observed. Employee is illiterate so having no education certificates to proof the details. PF office deny to take offline form for her claim. Employee is very much worried she is having financial crisis so she wants her PF claim.

Date of Birth :- 08/10/1972
The reason for the above variation may be attributable to clerical mistake on our part. In support of our statement containing Photo ID Beside, any of the following ID Proofs (as applicable) of the member are also enclosed in support of our statement (PAN card, Aadhar Card, Bank Pass book)
Kindly change name & DOB as per above. And process the PF advance withdrawal request.
