महोदय,
शिवप्रसाद पुत्र श्री रामचरन निवासी ग्राम गंगापुर भुलिया पोस्ट बेथरा तहसील कादीपुर जनपद सुलतानपुर राज्य उत्तर प्रदेश का स्थायी निवासी हूं मैं अपने यू0एन0 नम्बर 100488154536 से 10000 रू0 एडवान्स के रूप में आनलाइन फार्म भरा था अभी तक मेरे बैंक खाता सं0 3126000400336961 में नहीं आया है जबकि ईपीएफ का पासबुक चेक करने पर उसमें से 9900 रू0 कट चुका है
अत: महोदय से निवेदन है कि उक्त धनराशि मेरे बैंक खाते में भेजने का कष्ट करें
