I Smt. Ginidevi Sureka , W/O of late Mahabir Prasad Sureka hereby request you to change Gas Connection holder name ( Consumer No- 7107) in my son Lalit Sureka due to death of my husband. ( Death Certificate Xerox enclosed )

I also declare that I have no any objection in future for that change.

Please do the needful from your end & obliged.
