problem regarding with submission of bank details for DBTL

Dear Sir,

My self is Gor Kaushal P.

My consumers no. for bharat gas cylinders connection is 65453392 and my LPG ID is 1000000065453392.

I submitted request/application form duly filled all data to bank for DBTL scheme, which was forwarded to Jalmin GasDDistributer for verification.

I have recently received a message that, DBTL verification form has been rejected by gas distributor. Please let me know that why my form has been rejected even though I submitted form with all the proof.
