Sir,
With due respect, I beg to state following few lines for your kind information and favourable action pl.
That Sir, I was submitted documents for change of name and link in bank acct in gas connection (ID No 37500000024360905) on 15 Jan 2015 for smooth functioning of our gas connection, but my name will not yet changed.
Sir, I am serving in Assam rifles and posted in insurgency area of Nagaland and unable to come time to time for solve such types of problems. My family is staying in my home i.e. Salganga (Assam) and they faced difficulty for not obtaining the cylendar due to non change of name and not link in my bank account.
In view of the above you are requested to please change the name at the earliest.
