Sir,

1.Ref my letter dated 22 Apr 2015.

2. I was fwd an application for change of name in our gas connection and my family has suffer non availability of cylendar and they purchase cylendar from black.


3.Sir, I am serving in Assam rifles and posted in insurgency area of Nagaland & now the situation of Nagaland is very critical and unable to come time to time for solve such types of problems. My family is staying in my home i.e. Salganga (Assam) and they faced difficulty for not obtaining the cylendar due to non change of name and not link in my bank account.

4. Sir at my home only my wife and daughter is staying and no male persons is available at my home.

5.In view of the above you are requested to please change the name at the earliest.
