bank A/c not linked even after submitting documents.

Sir/Mam,
Even after submitting the proper requested documents to the bank and the distributor, my LPG connection has not been linked to the bank a/c as I asked in the bank. I have acknowledgement slip of document submission of distributor (MKB Indane) dated 13/12/14 and of bank (Union bank of India) dated 16/12/14.
As the last date of linking the a/c is nearing. Please see into the matter and do what's needed.
