Non receipt of account opening documents and bank account closure charges

I had opened saving account 5 months ago in the month of Jan 19 with Kotak Mahindra Bank. However i haven't received documents/ details as per account opening form (e.g. net banking ID password, phone banking, cheque book etc.) in spite of follow-up since last 5 months with the kotak bank's executive who had opened my account Ms. Pooja Jha - Cont No. 86554 04094. I had visited Mira road branch on 3rd June but even they couldn't help me and Finally i have to close my account as i was not able to operate my account. At the time of closure i was informed that there will not be any closure charges by Ms. Ramiza Ansari (Executive at Miraroad kotak bank branch). However i got message for my accounts got debited towards closure proceeds which was after deduction of closure charges of appx Rs.700 which was not intimated to me and not mentioned in the account opening form or closure of bank account form and therefore, I had visited Mira road branch on 7th June again, however staff at the branch has refused to reverse closure charges and therefore wanted to talk to the branch manager but in spite of waiting for approx. 2 hours he was not available at the branch. Refer all relevant evidences uploaded and wanted to know reason for not resolving customer complaint for such a long period of 5 months and harassing customer even after closure of account by charging bank closure charges for the fault of bank employees because of which i was not able to operate my account.
Spoke with again with Ms. Bhumi Parmar (Executive of Miraroad branch) on 10th June but she was also not able to help me in spite of i have told that i want to open new account and closed by service request no. 1-11433199681 without any resolution. I want refund of inappropriate charges for closure of accounts and compensation for loss of investment opportunities as i was not able to use my funds and mental harassment since last 5 months because of several followups.
