phone deactivation MTNL to reliance and submitted all the documents to Chembur office

Dear Sir,
I am a scientific officer working at BARC Mumbai. Under your new 599 plan I had opted for number portability from MTNL to reliance and submitted all the documents to Chembur office. As per the SMS received on 3.03.2016 my phone was activated under Reliance services.

The services were deactivated on 10th MARCH 2016 citing address verification. As advised by your customer I had again submitted all the documents on 11th but of no use.

Today my incoming was also disconnected I am not able to receive any SMS and not able to make any payment .This is causing great difficulty to me.
